The Lap-Band system: Fast
becoming a popular choice in the US, Lap Band is billed as the least invasive safest weight loss surgery. Lap-Band is wrapped around the stomach and creates an upper pouch. A narrowed outlet is created between the new upper pouch and lower stomach.
The diameter of the band can be adjusted through an access port. As the stomach outlet gets smaller, it helps reduce the amount of
food you eat and makes you feel full sooner. Surgery takes less than an hour and patients can usually return to normal daily activities
within just a few days!
Vertical Sleeve Gastrectomy (VSG): Also known
as “the sleeve”. This surgery once used as a first stage surgery in patients with a very high BMI, has now become a stand alone
surgery for patients wanting a procedure less invasive than the gastric bypass, yet they don’t want to or cannot commit to fills
and frequent follow up care that the Lap Band requires. This surgical weight loss procedure is used for severely obese individuals
weighing more than 100 lbs over their ideal body weight. The procedure has great promise.
The Roux-en-Y Gastric Bypass: This surgery is
a combination of a restrictive procedure (similar to the VSG) and a malabsorption surgery. The patient will be restricted to eating a
small amount of food and will not absorb all the calories consumed. A gastric bypass can be performed using laparoscopic surgery or
open surgery. Most surgeons prefer the laparoscopic approach. This surgery is known as the “Gold Standard” bariatric surgery in the
Untied States.
Mini-Gastric Bypass: This surgery is a modified version of
the Roux-en-Y Gastric Bypass. The stomach is fashioned into a long, narrow tube. The new small stomach is attached to the small intestine about
6 feet down. The larger stomach is sealed and left unattached. The Mini Bypass is done laporoscopically through five 1 inch incisions. The
simplicity of its construction reduces the challenge of laparoscopic surgery. The mini gastric bypass typically takes 30 – 40 minutes to
complete.
Duodenal Switch: The Duodenal Switch (DS) procedure, aka Biliopancreatic Diversion (BPD/DS), is composed of a restrictive and a malabsorptive aspect to achieve and maintain the best reported long-term percentage of excess weight loss. The restrictive portion reduces the stomach along the greater curvature, effectively restricting its capacity while maintaining its normal functionality. The malabsorptive portion rearranges the small intestine, creating two separate pathways and one common channel to separate the flow of food from the flow of bile and pancreatic juices.
Lap Band: You will be on a modified diet that includes liquids and soft foods for the first few weeks. Once you have moved on to solid foods, you should not consume liquid calories or soft calorie dense foods. The Lap Band will only restrict solid foods and not liquid calories. You will need to avoid sweets and junk food and should incorporate exercise in to your daily activities. You will need to refrain from drinking liquids ˝ hour before and 1 hour after meals. Non calorie liquids should be consumed between meals.
Vertical Sleeve Gastrectomy: You will be on a modified diet that includes liquids and soft foods and will progress to a regular diet. Your hunger will be greatly diminished but you will still need to avoid sweets and junk foods and should incorporate exercise in to your day.
Gastric Bypass: You will be on a modified diet that includes liquids and soft foods. You will progress very slowly to solid foods. You will need to consume non-calorie liquids between meals only. You may not drink any liquids ˝ hour before eating and 1 hour after meals. You should avoid sweets and high fat foods as they may cause you to feel ill and experience with is known as “dumping syndrome”. You should incorporate exercise in to your day.
Duodenal Switch: You may require three to four weeks of recovery after Duodenal Switch surgery. You can consume only fluids immediately following surgery. You will progress to pureed foods and ultimately solids. You may experience difficulty consuming liquids immediately after surgery because of swelling around the stomach and small bowel. Your surgeon will likely prescribe painkillers in the days after surgery. Your most weight loss occurs during the first 12 to 18 months after surgery. You can expect to lose about 70% of your current weight and about 35% of your BMI.

Generally in the United States, surgical candidates possessing a BMI of 40 and above or 35 with 2 co-morbidities such as diabetes, high blood pressure or sleep apnea. Most other countries already have clinical results for patients with BMI's between 30 - 35. All patients that stand to benefit should be considered for surgical treatment. Other factors, such as the number and severity of co morbidities, general health and well being are also evaluated.
